Clinic Resources

  • In 2015, the OAFC created Quality Care Standards to create dual accountability for clinics and the association in documenting, for public stakeholders, partners, and funders, the quality and quantity of services/care delivered by member clinics. The Standards document lists and explains the Quality Care Standards in detail.

  • In 2017, the Quality Care Standards were adapted into an electronic survey to make it more user friendly for our membership to use. In lieu of a tally sheet, the Standard Survey is answered in a yes and no format.

  • Along with the Quality Care Standards, the OAFC also created a toolkit of documents to help our clinics implement the standards.

  • AmeriCorps VISTA members serve non-profit organizations as capacity builders. They work on projects, write grants, recruit and train volunteers and much, much more. For more information on the VISTA program and hosting a VISTA, visit What We Do> AmeriCorps VISTA program.

  • Uninsured Care Funds are dispersed quarterly to clinics that were awarded funds during the last UCF application cycle. Reports are due quarterly to Beth at CHN, typically by the 12th of the reporting months (October, January, April, July). See the reporting dates in the document for details. To submit a UCF report, download and complete the UCF Reporting Document in its entirety and email to Beth or submit to the OAFC Dropbox.
