2020 Annual Survey

The 2020 Annual Survey, 2020 Rights and Responsibilities Forms, and additional documentation is due by March 14, 2020.

The 2020 Annual Survey will be submitted using Google Forms.  This platform is both user-friendly and collects information in a format that is easy for us to use.

  • A PDF of the survey is linked below, this can be used to review the questions ahead of time and prepare your answers before completing the survey. This is for viewing purposes only, only Google Form submissions will be accepted
  • To begin the survey and complete later, you need to ensure the first three questions are completed. Flip to the end of the survey and hit submit, you will then receive an email with a link that will allow you to return and complete the survey.

We are requesting several documents, which you can upload directly into the survey:

  • Your 2020 Board Roster
  • Your 2020 Budget
  • Your most recent Form 990 and/or Independent Audit
  • A copy of your logo and any promotional materials you would like to share

The last page of the survey has the links that you can easily upload these materials.


2020 Membership Packet

Once  your annual survey has been submitted, we will send an invoice for your membership dues. Regardless of when the packet is received annual fees are due by March 30, 2020. Dues are payable via credit/debit card or by check.  

The breakdown of Annual Membership Dues is unchanged from prior years and is shown below:
